Welcome

Several items are need to complete each process below.

Enrollment:

• Birth Certificate
• Immunication Record
• Unofficial transcript & last report card
• 2 utility bills (gas, electric, water)
• Driver's license
• Current IEP if SDC or RPS student
• California Exit Exam Scores

Transcripts:

• Name
• Date of birth
• Date of graduation
• College/University/Home address

Checking out of school:

• Return all books and sports equipment
• Take check out form to teachers for grades
• Parent to sign form
• Transcript/medical form will be given to parent when process is completed.
• Student's permanent file will be sent to the new school upon request.
