2017-2018 School Year
Athletic Clearance will be done online for the 2017-2018 school year, beginning May 1, 2017.
Below we have listed the website for you to use with step-by-step instructions. It is a very easy process to follow. We will not accept any papers in the office - it must be done online. The physical must be completed by a medical doctor (MD or DO) and dated after May 1, 2017. We will not accept physicals from a PA, NP, or Chiropractor. The form must be signed by the doctor and also include their stamp. Once you obtain the completed physical, you will need to upload it to the website below.

Register - Parents, register with valid email, username and password. You will then be asked to type in a code to verify you are human.
Select “New Clearance” to start the process.
Choose 2017-2018 School Year
Choose San Dimas High School
Choose a Sport
Complete all required fields for: Student Information, Educational History, Medical History, Insurance Information, Signature Forms, and Upload completed physical.
Once you reach the Confirmation Message, you have completed the process!
All of this data will be electronically filed with your school’s athletic department for review.
When the student has been cleared for participation, an email notification will be sent.
Online Athletic Clearance FAQ
Once you complete a clearance for one sport, most of the information you have entered will be retained in the system. To register for an additional sport, select New Clearance and after you enter the year, school, and sport most of your information will auto fill. It is a good idea to request clearance for all sports you might want to play so you don't have to wait for clearance later in the year.
The physical form can be downloaded here or a paper copy can be picked up in the front office. The physical must be completed by a medical doctor (MD or DO). We will not accept physicals from a Physicians Assistant, Nurse Practitioner, or Chiropractor. The form must be signed by the doctor and also include their stamp. Once you obtain the completed physical you will need to upload to the website.
